Quality Improvement

Quality Improvement (QI) is an important enabler for the Health & Social Care (HSC) system as it seeks to improve safety, reduce error and change the way services are delivered.
The HSC Safety Forum is a regional unit, sitting within the Public Health Agency (PHA) with a remit to support HSC organisations in their drive for QI and safety of patients/clients – this includes building QI capability and capacity.

The key aim of the QI ECHO is to provide staff in HSC Trusts with training in QI methodology while at the same time supporting the development and success of current trust-based QI projects.
This will include teaching on a framework for QI such as the Model for Improvement, using measures for improvement , PDSA cycles and understanding your system.

Dr Gavin Lavery is Clinical Director of the HSC Safety Forum and a consultant in Critical Care Medicine at Belfast HSC Trust.

Dr Lavery is a graduate of the Advanced Training Programme at Intermountain Healthcare and is a member of the Health Foundation’s College of Assessors. He is a standing member (Patient Safety) on Quality Standards Advisory Committee (#2) at NICE and a Board member at the Centre for Clinical Leadership and Quality in Healthcare at Royal College of Physicians in Ireland.

Previous roles include Lead Clinician for the Northern Ireland Critical Care Network, President of the Intensive Care Society of Ireland and an examiner for several colleges/faculties. Dr Lavery has published over 60 peer-reviewed papers and book chapters.

Levette Lamb is Regional Patient Safety Advisor with the Northern Ireland HSC Safety Forum, she is a Registered General Nurse who trained in the Mater Hospital in Belfast and went on to work in a range of nursing and management posts up until 2004.

Levette joined the service improvement unit at the Department of Health Social Service and Public Safety (DHSSPS) and led on a range of multiprofessional improvement projects across the region. In 2009/10 Levette was asked to develop the Nursing and Midwifery strategy for Northern Ireland. Entitled “A Partnership for Care” . She has completed the Patient Safety Executive and Improvement Advisor programmes through the Institute of Healthcare Improvement. Levette is a founding member of the Health Foundations Q Programme.

Janet Haines-Wood is a Regional Patient Safety Advisor with the HSC Safety Forum. She began her career in quality improvement in the early 1990’s working in a range of roles from clinical audit manager, Project Manager for the King’s Fund Organisational Audit and clinical and social governance manager in both HSC Trusts and Prison Healthcare.

She joined the HSC Safety Forum in July 2009. She has completed the Patient Safety Executive Development Programme and the Improvement Advisory Programmes associated with the Institute for Health Care Improvement completed the Clinical Microsystems Programme (Dartmouth Institute.)

Programme Details

The programme will run from October 2016 – April 2017. Two streams of the programme will take place concurrently and shall cover the following topics:

  • Where do I start?
  • Supporting Change
  • Develop and Test a Change
  • Moving from Testing
