SWAH "Mind the Gap" ECHO

SWAH "Mind the Gap" ECHO

This ECHO Network is an Interface between primary and secondary care in the South West area of the Western Trust.

The aims of this network are to improve;

• Discharges and admissions
• Outpatient efficiency
• Communication, and understanding between primary and secondary care services relating to the S.W.A.H.
• Helping to co-produce and co-design service models which work in the face of Covid-19 and the pending challenges of seasonal flu.

This network, “Mind the Gap” is all about strengthening relationships between primary and secondary care, sharing key information, and experience, so that there is support, learning, and peer to peer encouragement in delivering care to the population of the southern sector of the Western Trust.

It is based on the everyone a teacher everyone a learner model of ECHO, topics are chosen by the spokes, and it responds to raised needs from the spokes.
