    Overview

    About this network

The Care Home Support Team is a Southern Trust multidisciplinary team made up of nursing, social work, occupational therapy and administrative staff. It supports the care of older people who are residents of the Trust in permanent nursing and residential home placements.

The team works in partnership with Care Home staff, who work closely with the RQIA, Integrated Care Teams for Older People, Memory Services, Governance and Integrated Care Partnerships (ICP). The Care Home Support Team also provides education and training programmes for Care Homes, and is part of the regional Care Home Transformation project.

The aim of the ECHO network is to support staff through continued education, to help reduce avoidable unscheduled hospital attendances and admissions, and to help with understanding all appropriate pathways for residents.

Through the ECHO model, the Care Home Support Team can engage with more nursing home staff, as staff can attend sessions from their own workplace. Sessions are recorded, so staff can access the training at a time that suits them.
